Flight Exam Information:

Before your exam, you must:

  • Go to https://medxpress.faa.gov
  • Complete the FAA MedXpress patient information portion of the exam on-line prior to making your appointment.
  • Bring the following documentation:
    • Your FAA MEDXPRESS CONFIRMATION NUMBER.
    • Pertinent medical documentation as required for previous special issuances, such as; medical records, lab results, signed and completed FAA medical forms, e.g., Form 8500-7, personal or specialist physician letters and documentation, Statement of Demonstrated Ability (SODA), etc.
    • Your corrective lenses, contact lenses with case, or glasses.
    • Please read the Pre Medical Checklist on our website for additional items.

  • Payment is DUE AT THE TIME OF SERVICE for all Aviation Medical Exam physicals.

Your Pre-Medical Checklist:

  • Go to the **FAA website: https://medxpress.faa.gov/** ,and complete the Form 8500 accurately. Print a copy of Form 8500 and bring it with the FAA MED-XPRESS CONFIRMATION NUMBER provided, to your appointment.
  • Bring a picture ID (i.e. Drivers license, passport).
  • Bring your current medical certificate.
  • Your medical examination will include an eye exam. If you wear eyeglasses, please bring them at the time of your examination. If you wear contact lenses, please wear them for your examination.
  • If anything regarding your medical history has changed since your last medical certification exam, please bring a copy of all pertinent medical information, such as; medical records, physician office notes, signed laboratory results, completed FAA medical forms (e.g. Form 8500-7), consultation reports, hospital records and any procedures or operative reports.
  • If you are currently taking Coumadin as one of your medications, please bring a copy of your PT/INR (Protime/International Normalized Ratio) results for the past six months.
  • If you are diabetic please provide a log of your blood sugar results, including HgbA1c (Hemoglobin A1C) for the last 3-6 months.
  • If you have a SODA (Statement of Demonstrated Ability), please bring a copy at the time of your appointment.
  • Bring a list of all of your medications along with the following:
    • Name of Medication
    • Dosage
    • Frequency of use (once a day, twice a day, only when needed, etc)
    • Condition which requires you taking the medication
    • Date medication initially prescribed
    • Adverse effects if any, you have experienced, as a result of taking the medication
  • Print this page and use it to prepare for your medical exam.
